Agreement and Release Form:

At your first orientation meeting, you will receive a copy of the Agreement and Release form. Sign this form in the presence of a notary. UD students can have the A&R form notarized for free in the Student Services Building. Students from other universities should ask their study abroad advisor whether their campus has a similar service. Bring (or mail) the completed, signed, and notarized form to your faculty director at your second orientation meeting.

A copy of the Agreement and Release form is available FOR READING ONLY here as a PDF document, but DO NOT PRINT this version. It will not be accepted by CFIS. Only the original front/back copy, distributed at your orientation meeting, may be signed, notarized, and submitted.
